Direct Access?
With Direct Access care at LEAP, we will provide comprehensive primary care services for acute, chronic, and wellness issues on an ongoing basis. We will collaborate with you to develop a personalized care plan tailored to your needs, based on thorough discussions and interactions with your provider.
We offer flexible meeting options, including in-person consultations and telemedicine appointments where appropriate. Our communication channels include in-person visits, patient portal, phone, and secure email. We are dedicated to understanding your needs and concerns, and we will work diligently with you to enhance your health and well-being.
